I consent to my child(ren) or ward’s participation in AIM Debate Camp. I realize that there are risks of bodily harm inherent within the camps activities, including but not
limited to organized games and free time activities. I hereby agree to indemnify and hold harmless AIM, its volunteers, or agents for any loss or damage through personal injury or otherwise whether or not the loss or damage is caused by negligence of AIM its volunteers, or agents, and claims arising from any accident or sickness to my said child (ren) or ward while participating in the said
program.
COVID-19 While AIM and Zion Spokane are going to great lengths to comply with state
and county guidelines I recognize that there is a potential risk of COVID-19 exposure. I release AIM and Zion Spokane and I assume the risk of potential COVID-19 exposure while attending camp. I understand that I may be asked health questions daily, but that masks and vaccines will not be required to attend camp.
In case of emergency, I understand that every effort will be made to contact me; however, I hereby give my
permission to the physician selected by the camp to hospitalize and secure proper treatment for my child(ren) or ward named above.
Photo and Indemnity Release
I agree to the Photo and Indemnity Release
I hereby grant permission to AIM and Zion Spokane, to use my child(ren) or ward’s photograph on websites or social media or in official printed publications without further consideration, and I acknowledge AIM's right to crop or treat the photograph at its discretion. I also acknowledge that the Institute may choose not to use my photo at this time, but may do so at its own discretion at a later date.
I hereby grant AIM permission to use my child’s name in future advertisements based on any future success with Stoa Tournaments or NITOC.
I also understand that once my child(ren) or ward’s image is posted on AIM website, the image can be downloaded by any computer user. Therefore, I agree to indemnify and hold harmless from any claims the following:
